CTY works with families so that students with allergies, asthma, and/or diabetes can safely attend our day and residential courses. Protocols are handled based on individual needs.
Asthma Action Plan
Download the attached Asthma Action Plan and complete it with your physician along with the Physician Authorization pages available for download in the Physician Authorization section of your CampDoc account. Once completed, upload it to your CampDoc account. An asthma action plan is not required. You may submit any asthma care plan your child has on file from the regular school year.
Allergy Action Plan
Download the attached Allergy Action Plan and complete it with your physician along with the Physician Authorization pages available for download in the Physician Authorization section of your CampDoc account. Once completed, upload it to your CampDoc account. An allergy action plan is not required, unless your child needs to carry an EpiPen or other allergy medication with them at all times. You may submit any allergy care plan your child has on file from the regular school year.
Diabetes Management Plan
Submit a Diabetes Management Plan for your child to have on file with the site nurse. The Diabetes Management Plan requires a doctor’s signature. All medications that your child is taking, including insulin, must also be entered on their CampDoc CTY Medical Form. You must also request accommodations for your child to ensure they can provide self-care as needed. Learn more about requesting accommodations for CTY testing and courses.
If you have used CampDoc with CTY in previous years, you can navigate directly to CampDoc’s website and sign in to access medical forms. If you do not remember your password, click the forgot password button and enter the primary email address on your MyCTY account.
If you have not used CampDoc with CTY, you will receive an invitation to create an account after enrolling in a course. To access the medical form, click the “Accept Invite” button in the email from CampDoc.
